Situated in Kotahana, the temple was originally created by Sir Ponnambalam Muthaliyar and rebuilt by his son Sir Ponnambalam Ramanathan. The first Kumbavishagam was held in 1857. This temple is one of the prominent temples in Sri Lanka, with the main idol being Lord Shiva in lingam form. The annual water cutting ceremony takes place on Panguni Utiram, and 10 days before that, the flag hosting ceremony occurs. The chariot festival takes place before the water cutting ceremony. Currently, the temple is worshipped by both Hindus and Buddhists who venerate Lord Shiva.
